Home and Community Based Services
Home and Community-Based Services, or HCBS for short, offers individuals the opportunity to build essential daily living skills in the comfort of their own home or community. Springbrook currently supports more than 220 individuals enrolled in Respite Services and Community Habilitation across Chenango, Broome, Delaware, and Otsego counties.
Individuals work to realize the goals set in their personalized habilitation plan. These could include goals related to personal care, social skills, household chores, community integration, budgeting money, cooking skills and more. These goals are determined by the individual, as well as their family. It is the intention of HCBS to assist the individual they serve to become as independent as possible.
Services are typically provided one on one, which truly allows both the staff and the person they are working with to learn from one another. Individuals are matched with HCBS staff based on the goals and needs of the participant. As the individual gains and develops skills, staffing can change so as to most effectively support the individual’s growth.
