Self-Directing your services with Springbrook ensures flexibility to choose the supports that are right for you and your family, including the staff and schedule of your choice, and the technology and expertise to support you every step of the way. 

  • You can choose Self-Directed Services if you are eligible for OPWDD services and enrolled in the Home and Community-Based Services (HCBS) Waiver.
  • Choosing to Self-Direct your services with Springbrook means you are partnering with one of New York State’s leading SDS providers with over 15 years of experience helping individuals and their families to build the life they want.
