The School at Springbrook

Springbrook's school program serves children with developmental disabilities ages 5 to 21, whose primary diagnosis is Mental Retardation. The school provides a residential or day program.

All referrals to the school program must be made by the CSE (The Committee on Special Education) of the home school district of the student.   Sprinbrook has Admission, Discharge, and Preclusion Criteria. Click here for residential and click here for day students.

If the home school district believes that a student would be best served at our residential school, the district must submit a referral packet containing current information about the student to Springbrook.

The packet should include:

  • Current Individualized Education Plan (IEP)
  • Current Psychological Evaluation
  • Records from previous residential placements
  • Social History
  • Medical records including history, dental, immunizations, medication, and copy of last physical exam
  • All records pertaining to behavioral issues including data collection, progress notes, intervention plans
  • Individual Service Plan (ISP) (if available)

Students placed in a residential or hospital setting: If a student is currently in another residential or hospital setting, Springbrook needs to receive records from the facility. All records for the previous 3 months should be included with the referral packet. Students will not be considered if these records are not received.

This packet of information will be reviewed by Springbrook. If Springbrook feels that our program may be able to meet the needs of the student, a screening and evaluation to assess the student will be arranged.

Springbrook believes in a team approach and requires that the school district and parents and/or guardians are involved in the referral and admission process.

The Community Homes at Springbrook

Springbrook operates 20 homes in Otsego, Chenango and Madison Counties which are designed to support individuals with developmental disabilities who are over 21 years of age. Our homes are staffed 24 hours per day and are located in community settings with access to recreational, social and work opportunities.

Admission will be considered for individuals whose primary diagnosis is Mental Retardation and whom meet service eligibility requirements as defined by the NYS Office of Mental Retardation and Developmental Disabilities. Upon contact with Springbrook's Director of Community Homes, an application for admission packet will be sent to the referring person.

Information sent to Springbrook should include:

  • Individualized Education Plan (IEP) and/or Individual Service Plan (ISP)
  • Psychological Assessment (within the past year) and/or a Psychiatric Assessment
  • Copy of last physical
  • Materials from previous placements
  • Social History
  • Medical records including history, dental, immunizations, medication
  • Behavioral data

If the initial review of materials indicates that the individual may be appropriate for placement at Springbrook, a personal screening will be arranged.
